PA DBE U Program Participation Overview:
Below are all the necessary forms and instructions for participation in PA DBE University. For DBE firms that are new to PA DBE U, please start at number one and submit a Participation Agreement for the current fiscal year. For firms that have already submitted a Participation Agreement for the current fiscal year, you may begin submitting Reimbursement Requests.
1. PA DBE University Participation Agreement:
· This form must be completed and returned prior to any participation in PA DBE University.
· This form must be printed, signed and returned via fax to (610) 399-2118 or scanned and emailed to penndbe@cheyney.edu.

2. PA DBE University Approved Course List:
· Review the approved course list and select courses for reimbursement using the Reimbursement Request Form.
· If the course you would like to request reimbursement for does not appear on the approved list you may submit a request for a new course/provider to be added/approved using the
PA DBE University Course Approval Request Form.

3. PA DBE University Training Reimbursement Request Form:
· Once a course/training has been completed and payment satisfied, you must be complete this form for every employee and every course you are requesting reimbursement.
· Proof of completion and proof of payment for the training MUST be included when submitting this form.

4. PA DBE University Training Satisfaction Survey:
· This needs to be complete immediately after you submit your PA DBE University Reimbursement Request.
· For every course that is completed, a survey must be submitted for each employee who attended course sessions. Separate surveys are required for every employee in each course.
