Below are instructions for proper completion of the PA DBE University Reimbursement Request Form. If you are ready to download and begin completing a new Reimbursement Request, please click the link below.

This form is an interactive MS Excel document and is designed to be completed electronically and emailed back to the DBE Supportive Services Center at penndbe@cheyney.edu.

  • Once all courses have been completed, your firm must submit a PA DBE Training Reimbursement Request Form. Please complete the entire form and provide all necessary info for every employee who has fully completed and paid for training.
  • Please list every individual employee who has completed any instance of training.
  • Multiple employees can be listed on the same form.
  • The same employee can, and should, be listed multiple times if he/she attended multiple trainings.
  • Please Do NOT include employees who have not yet fully completed training courses. Only include employees who have fully completed trainings and have the necessary supporting documentation.
  • Your firm will need to use the course numbers from the PA DBE University Course List, and enter them onto the form in the PA DBE Course # column. Some other necessary course information fields will be pre-populated once the course number is entered.
  • Please do NOT add new employees to an already existing and submitted reimbursement request.
  • It is very important to always use the most current version of this form. You can check revision dates in the top left corner of the Instructions tab in this form. Always download a new form when submitting a new request. Do NOT use an old version of this form for reimbursement. (You may find that new courses are not visible on an older Reimbursement Request form.
  • If your course has been approved but does not appear in the drop down list of the course number column, please download a new Reimbursement Request form. If your course does not appear on the most current version of this form, please contact the DBE SSC immediately at (877) Penn-DBE. 
  • Once all appropriate information has been entered onto the form; please save a copy for your records and email back to the DBE SSC @ penndbe@cheyney.edu.
  • Once you have submitted a reimbursement request, that request will be processed and you will be notified of any change in status.
  • In addition to the reimbursement request the following documents MUST also be submitted prior to reimbursement being issued:

      -  An invoice from the training provider showing proof of payment or a copy of both sides of a cancelled company check for payment.
      -  A certificate of completion or a signed statement from the training provider indicating that the employee completed the training.
